What’s the difference between the EPO and PPO plans?

Exclusive Provider Organization (EPO) Plan - Just the Basics

This plan does not provide any benefits for out-of-network services unless network services are not available or for:

Emergency services.
Services from facility-based providers in a network facility.
Laboratory service providers when services are provided in connection with a service from a network provider.
Diagnostic imaging services from an out-of-network provider when services are provided in connection with a service from a network provider.

*For states where an EPO is not allowed, the Curative PPOx Plan will be available. PPOx will be offered in AR, AZ, HI, MS, MT, ND & OH. PPOx has the same structure as an EPO.

Preferred Provider Organization (PPO) - The Classic Package

Members choose where to go for care, without a referral from a primary care physician or having to only use providers in your plan’s provider network. Care from out-of-network providers will be subject to the out-of-network deductible.

Out-of-network Deductibles: $10,000/person; $20,000/family

Out-of-network Max Out-of-Pocket: $15,000/person; $30,000 family

Preferred Provider Organization (PPO+) All the PPO benefits, with a lower threshold to afford out-of-network providers. Includes credits to ClassPass, an app with fitness classes, gyms, and wellness services. More features, such as additional coverage, will be added as the product continues to develop.

Out-of-network Deductibles: $5,000/person; $10,000/family

Out-of-network Max Out-of-Pocket: $7,500/person; $15,000 family

Is there a spousal surcharge if I choose to cover my spouse under the Curative plan but they have medical insurance offered to them through their own employer?

There is no spouse surcharge and spouse can be carried on the plan even if they have covered through their employer.

What perks does Curative have for employees?

Curative offers no copays and no deductibles for all members who complete the Baseline Visit within the first 120 days of their plan’s effective date. Additionally, Curative members have access to multiple health and wellness services including 24/7 telehealth, weight management support, and substance use management support. How does Curative handle pre-existing conditions?

Curative does not discriminate against pre-existing conditions. Our plans cover all members equally, ensuring that everyone receives the care they need from day one. This means that your clients can feel confident knowing their employees are fully covered, regardless of their health status.

Question 7

What if my in-network doctor refers me to an out-of-network lab for bloodwork?

When a member is referred by an In-network provider to an out-of-network lab for testing the claim for the lab testing will be processed at the in-network benefit levels as the request was originated by the in-network provider.

Question 8

What is the process, timeframe and general success rate there is for out-of-network providers to be brought in-network?

Accepted Answer

We actively try to bring out-of-network providers requested by members into the network. The process can take up to 120 days. A member requests a provider be in network. We pass it on to the recruitment team who gets in touch with the provider and send a contract. The most common delay in onboarding is a response from the provider, but we do everything we can to speed up the process.

What if I am currently under doctor’s care for a chronic condition and really like my doctor but they are not in the Curative network?  How will that be handled?

Curative has a transition of care process for members under critical care. We ask that members contact member services or their care navigator to be routed to case management to discuss the process of transitioning to an in-network provider or qualifications to retain their current provider.

Will the Care Navigators help transition to in network care?

Accepted Answer

The Care Navigators can assist in transitioning members to in-network providers. This conversation will be prompted at the Baseline Visit, but you may reach out to your Care Navigator or Member Services at any time.

Question 11

If I am currently on medications and they are not covered by Curative, how will that be handled?

Accepted Answer

Curative’s preferred drug list covers many popular generic, brand name, biosimilars, specialty, select brand and over-the-counter drugs.
Non-preferred drugs have a $50/$250 Copay. Pharmacy team members are also on the look-out for any cost-saving opportunities for our members. We’ll work with members and their doctors to keep your copay costs low.

Which pharmacies are in-network for Curative?

Curative provides access to a nationwide network of pharmacies including the Curative Pharmacy, Albertsons Pharmacy Companies (Including retailers such as Randalls and Safeway), H-E-B, Brookshire Brothers & Publix Pharmacy.

How do I transfer and fill my current my prescriptions?

You may transfer and fill your prescriptions by visiting curative.com/pharmacy for the form or call Member Services for assistance. You can transfer before or at your Baseline Visit, depending on timing of needs.

How do I access my prescriptions at the lowest possible cost?

How to qualify for $0 copay in three easy steps:

Get your Baseline Visit within 120 days of your start date.
Visit an in-network provider who prescribes a preferred medication.
Use our preferred in-network pharmacies.

Preferred:

Consists of generic, biosimilars*, select brand and specialty medication

$0 Copay

Non-Preferred:

Consists of lower value generics, brands, and specialty medications

$50/$250 Copay

*A biosimilar is a biological product that is highly similar to and has no clinical meaningful differences from an existing FDA-approved reference product. They are used to decrease healthcare costs and improve patient access.

How soon should I book my Baseline Visit?

We recommend booking your Baseline as soon as possible to ensure your appointment takes place within 120 days of the plan's effective date. Completing your Baseline will ensure you will continue to qualify for $0 copays and a $0 deductible for the plan year.

What is a Care Navigator?

Your Care Navigator is your go-to source for all things Curative and will be your direct point of contact if you have questions or concerns about your coverage. Your Care Navigator can help you find providers and navigate your benefits to help you reach your health goals. You do not need to wait until the Baseline Onboarding to connect with your Care Navigator. Go to your Member Portal to find their information.

Question 17

How are Baseline Visits conducted?

Baseline Visits will be conducted virtually over Zoom. While we strongly recommend completing your Baseline Visit over a computer, if you join using a mobile device, you must download Zoom prior to your appointment. Members who access the Baseline Visit over a computer have the choice to join through a browser and not download Zoom.

It is strongly recommended that cameras are on during the Baseline Visit.

Is there bloodwork at the Baseline Visit?

Bloodwork can be ordered if it is deemed medically appropriate.

Question 19

If there are multiple members of the family that are 18 years or older and one person doesn’t get the Baseline Visit, is everyone else in the family penalized?

Accepted Answer

No, only the person(s) that do not complete their Baseline Visit will be subject to the in-network non-Baseline Visit compliant plan.

All members 18 or older must individually complete their own Baseline Visit to qualify for $0 copay, $0 deductible in-network care and preferred prescriptions.
Only one adult member has to complete a Baseline for a minor dependent to qualify.
An incomplete visit will result in a $5,000 deductible ($10,000 family) and copays for in-network care.

Question 20

Does the Baseline Visit have to be redone at the beginning of every plan year if you have already completed one?

Yes, we ask that they complete a Baseline Visit each year.

If I complete the Baseline Visit after the first 120 days, will the $0 copay and $0 deductible be reinstated?

Accepted Answer

It will not. The Baseline Visit must be completed within the first 120 days in order to unlock the $0 copay and $0 deductible benefits.
