Curative Cash Card
The Curative Cash Card, formerly the Curative Zero Card, helps members get $0 care when and where they need it. Members can use it with providers labeled “Cash Card only” in the Curative Provider Search. For in-network providers, members should use their Curative Member ID card. Members can also use their Cash Card at any urgent care location. . Members use it like a payment card, but they do not pay anything.

We offer $0 copays and deductibles for any doctor in our search.
1) Use your Curative Member ID Card for in-network providers, standalone labs, and prescriptions.
2) Use your Curative Cash Card only with providers labeled “Cash Card only” in the Provider Search.
If you are unsure which card to use, contact Member Services before your appointment to help avoid payment issues.
Option 1: Member ID Card
Use your Curative Member ID card when your provider is listed as in-network in the Curative Provider Search. You should also use your Member ID card for standalone labs and prescriptions.
Option 2: Curative Cash Card
Use the Cash Card only for providers labeled “Cash Card only” in the Curative Provider Search. At the front desk, say you will pay the cash price and hand over your Curative Cash Card.
- Eligible services: Primary care, office visits, urgent care, behavioral health, and many inpatient and outpatient hospital services.
- Ineligible services: Prescriptions, labs, other non-covered benefits.
Add the Curative Cash Card to your Apple Wallet
Access the Member Portal
Open the wallet app and sign in with your Apple ID if required.
Tap the "+" sign and choose "Debit or Credit Card."
Enter the Curative Cash Card details manually or position your physical Curative Cash Card in the frame to scan
Add the Curative Cash Card to your Google Pay
Access the Member Portal
Open the Google Pay app and tap "Add to Wallet" at the bottom. Then, select "Payment card."
Tap "New credit or debit card" and tap "Save" at the bottom.
Read the Issuer Terms and tap "Accept." If prompted to verify your payment method, choose an option from the list.
Cash Card for urgent care
You can use your Curative Cash Card at any urgent care location. If the urgent care is in-network, you also have the option to use your Member ID card.
Planned surgeries or procedures
As soon as you’re aware of a planned surgery or procedure, contact Member Services at 855-428-7284 so we can help make sure everything is set up and let you know if a good faith estimate is needed from your provider.
The Curative Cash Card is available to all members during their first 120 days on any Curative Health plan, and for the full year to members who have completed their Baseline Visit during the first 120 days. Curative Cash Card Visa® Commercial Credit cards are issued by Celtic Bank. Additional Terms & Conditions can be found in your Member Portal Account.

Members must complete their Baseline Visit in the first 120 days of their plan start date to maintain $0 out-of-pocket costs.
The Curative Cash Card can be used only when Curative is the member’s primary insurance.
It counts as payment in full and does not work with other insurance.
Cash Card payments are limited to eligible, covered services and are subject to plan rules.
Transactions may be reviewed for coverage, clinical guidelines, usual and customary compliance, and may require prior authorization.
Provider participation, payment methods, and acceptance of new patients may change at any time.
