A Baseline is your opportunity to learn how to maximize your benefits and create a health roadmap. There are two required parts: the Baseline Onboarding and the Baseline Clinical Check-in.

Part 1: onboarding

Learn about your plan’s benefits and resources, find $0 providers, sign up for telehealth, check medication coverage, and connect to programs to reach your health goals. You’ll have two options to onboard:

  • Self-guided onboarding

    • Watch videos and complete important tasks at your own pace.

    • Your Care Navigator will follow up to answer any questions.

  • Live onboarding

    • Schedule a virtual Zoom meeting with your Care Navigator to walk through your plan in real-time.

Part 2: clinical check-in

This session is a one-on-one Zoom meeting with a Curative Clinician to discuss your medical history, address health questions, and create a tailored plan for your needs.
