Find your match
Finding your favorite care providers is important to your long term health. EPO, PPO, and PPO Max Members: Use our provider network search tool. EPO Value Members: Sign in to the Member Portal and open the Curative Pass tool to search for the care you need. You can also reach out to your Care Navigator or Member Services at 855-428-7284 and they will be happy to serve you.
Bring your provider with you
We know how important it is to have a provider you trust. Here’s how to connect with the right care based on your plan type:
- EPO Value Members: The Curative Cash Card gives you access to $0 care at providers labeled “Cash Card only” in the search tool and for any urgent care location. For in-network providers, use your Member ID card. If you are unsure which card to use, contact Curative before your visit.
- All other Curative plan members: You can use the Provider Search tool to find high-quality, in-network providers for $0 care.
If your provider isn’t already in network, you can nominate them to join the Curative Cash Card network using the form below.
