Step 1: Get connected.
To see if this program is a right fit for you, attend your Baseline Visit within 120 days of enrollment or contact member services at (855)-4-CURATIVE (855-428-7284).
Step 2: Learn where you stand.
Once enrolled, you’ll learn the ins and outs of your condition (everything from your diabetes ABC’s to which nutrition plans are best for you.)
Step 3: Ongoing care.
From there, we’ll put together an action plan that’s right for you. You’ll receive personalized goals and periodic check-ins to monitor your numbers and go over any concerns along the way.
