Applies when we mention $0 copays, $0 deductibles, or $0 out-of-pocket costs.

Members maintain $0 out-of-pocket costs for covered care and preferred prescriptions by completing an annual Baseline Visit within 120 days of their plan start date.

Pharmacy and drug formularies

Applies to Curative Pharmacy, preferred prescriptions, and our drug formularies.

Plans

EPO Value

Applies to the EPO Value plan and the Curative Pass.

Members maintain $0 out-of-pocket costs for covered care and preferred prescriptions by completing an annual Baseline Visit within 120 days of their plan start date. EPO Value members must use a Curative Pass.

PPO Max

Applies to out-of-network care on the PPO Max plan. The $0 care disclaimer also applies.

Curative covers the copay and deductible, but OON doctors might charge more than usual and customary fees and if they do, you might have to pay the difference.

ASO (self-funded plans)

Applies to employers with a self-funded plan administered by Curative.

Members maintain $0 out-of-pocket costs for covered care and preferred prescriptions by completing an annual Baseline Visit within 120 days of their plan start date. The Administrative Services Only (ASO) plan option provides self-funded coverage administered by Curative. Stop-loss coverage and administrative services are subject to specific contractual terms and compliance guidelines.

Curative Cash Card

Cash Card

Applies to the Curative Cash Card on all plans except EPO Value. EPO Value members, see EPO Value Cash Card.

*Members must be 18 years and older to use the Curative Cash Card and complete their Baseline Visit in the first 120 days of their plan start date to maintain $0 out-of-pocket costs. Curative Cash Card Visa® Commercial Credit cards are issued by Celtic Bank. Additional Terms & Conditions can be found in your Member Portal Account at health.curative.com. This overview does not apply to EPO Value members. EPO Value members should follow the Curative Pass guidance in the Member Portal to confirm when to use their Cash Card.

**For limitations of the Curative Cash Card, go to: curative.com/cash-card. The Curative Cash Card can be used only when Curative is your primary insurance. It counts as payment in full and does not work with other insurance.

***All Cash Card payments are limited to eligible, covered services and are subject to plan rules. All transactions are reviewed for coverage, clinical guidelines, usual and customary review compliance, and may require prior authorization. Provider participation, payment methods, and acceptance of new patients may change at any time.

EPO Value Cash Card

Applies to the Curative Cash Card for EPO Value members.

Members must be 18 years or older to use the Curative Cash Card and complete their Baseline Visit within the first 120 days of their plan start date to maintain $0 out-of-pocket costs. Members must also obtain a Curative Pass before receiving in-network care, except for urgent care. Curative Cash Card Visa® Commercial Credit Cards are issued by Celtic Bank. Additional Terms & Conditions can be found in your Member Portal Account at health.curative.com.

Curative Primary Care

Applies to Curative Primary Care and its clinicians.

Curative Primary Care is a physician-owned medical practice and its clinicians exercise their own independent medical judgment. Its administrative operations are managed by an affiliate of Curative Inc.

Applies to Curative Telehealth, virtual care, and mental health support.
