Our approach begins with a Baseline Visit—completed within the first 120 days of enrollment—which continues $0 out-of-pocket costs for in-network care and preferred prescriptions for the rest of the plan year. This visit combines a clinical check-in and onboarding with a Care Navigator to create a personalized health roadmap.
With a range of plan designs, employers can select the option that best fits their team’s needs, location, and budget.

Curative Plan Options
EPO Value Plan – Our most affordable option
Value Plan: $0 out-of-pocket costs when employees see Curative-recommended in-network providers with a Curative Pass.
In-network: Wide choice of nearly 1M providers, with $0 costs only when using a Curative Pass.
Out-of-network: No coverage.
Pharmacy network: Mail order and select retail (H-E-B, Albertsons, Safeway, Publix) with 30,000+ pharmacies nationwide.
Fitness: None.
Best for employers who want to minimize premiums while guiding employees toward high-value providers.
EPO Plan – Essentials at a lower cost
Essentials Plan: $0 out-of-pocket costs limited to in-network coverage only.
In-network: Wide choice of nearly 1M providers.
Out-of-network: No coverage.
Pharmacy network: Mail order and select retail (H-E-B, Albertsons, Safeway, Publix) with 30,000+ pharmacies nationwide.
Fitness: None.
Ideal for employers who want broad in-network choice without paying for out-of-network flexibility. Curative Pass not required as is on the EPO Value plan.
Note: Employers choose EPO Value or EPO, not both.
PPO Plan – Choice and flexibility
Choice Plan: $0 out-of-pocket costs in-network, with flexibility to use out-of-network providers for some cost.
In-network: Wide choice of nearly 1M providers.
Out-of-network: $10k/$20k deductible (individual/family).
Pharmacy network: Mail order and select retail (H-E-B, Albertsons, Safeway, Publix) with 30,000+ pharmacies nationwide.
Fitness: None.
Perfect for employers who value out-of-network access but still want to keep premiums moderate.
PPO Max Plan – Maximum choice and coverage
Maximum Choice Plan: $0 out-of-pocket costs for both in- and out-of-network care across the U.S.
In-network: Wide choice of nearly 1M providers.
Out-of-network: $0 deductible / $0 coinsurance.
Pharmacy network: Includes CVS, Walgreens, Walmart, Rite Aid, plus 60,000+ pharmacies nationwide.
Fitness: ClassPass included.
Ideal for employers seeking ultimate flexibility and access for employees.
