What is Curative?

[Curative](https://curative.com) is a health plan focused on helping you stay healthy, not just treating you when you're sick. After completing an annual [Baseline Visit](https://curative.com/baseline), you’ll continue with access to $0 copays, $0 deductibles, and $0 out-of-pocket costs for in-network care and preferred prescriptions.

What’s the difference between the EPO and PPO plans?

__Exclusive Provider Organization (EPO) Plan - Just the Basics__

This plan does not provide any benefits for out-of-network services unless network services are not available or for:

Emergency services.

Services from facility-based providers in a network facility.

Laboratory service providers when services are provided in connection with a service from a network provider.

Diagnostic imaging services from an out-of-network provider when services are provided in connection with a service from a network provider.

**For states where an EPO is not allowed, the Curative PPOx Plan will be available. PPOx will be offered in AR, AZ, HI, MS, MT, ND & OH.*

__Preferred Provider Organization (PPO) - The Classic Package__

Members choose where to go for care, without a referral from a primary care physician or having to only use providers in your plan’s provider network. Care from out-of-network providers will be subject to the out-of-network deductible.

__Out-of-network Deductibles:__ $10,000/person; $20,000/family

__Out-of-network Max Out-of-Pocket:__ $15,000/person; $30,000 family

My spouse or child also works for TEAM, can I carry them under my Curative medical insurance as a dependent?

Yes

Question 4

Is there a spousal surcharge if I choose to cover my spouse under the Curative plan but they have medical insurance offered to them through their own employer?

Accepted Answer

There is no spouse surcharge and spouse can be carried on the plan even if they have covered through their employer.

Question 5

I am currently on the TEAM HSA medical plan. Can I continue to use my HSA funds?

Accepted Answer

TEAM will not be offering an HSA plan option in 2024, meaning they cannot contribute to an HSA account through payroll deductions, and TEAM will not make any seed money contributions to HSAs.   You can no longer contribute to the HSA plan, However, any money that you have in an HSA account is yours to keep and you can continue to use those funds for out-of-pocket medical, dental and vision expenses in 2024 and beyond until the HSA funds run out.

How does Curative handle pre-existing conditions?

Curative does not discriminate against pre-existing conditions. Our plans cover all members equally, ensuring that everyone receives the care they need from day one. This means that your clients can feel confident knowing their employees are fully covered, regardless of their health status.

Question 7

If I am currently on medications and they are not covered by Curative, how will that be handled?

Curative’s preferred drug list covers many popular generic, brand name, biosimilars, specialty, select brand and over-the-counter drugs.  
Non-preferred drugs have a $50/$250 Copay. Pharmacy team members are also on the look-out for any cost-saving opportunities for our members. We’ll work with members and their doctors to keep your copay costs low.

Question 8

What if my in-network doctor refers me to an out-of-network lab for bloodwork?

Accepted Answer

When a member is referred by an In-network provider to an out-of-network lab for testing the claim for the lab testing will be processed at the in-network benefit levels as the request was originated by the in-network provider.

Pharmacy Network

Curative provides access to [nationwide network of pharmacies](https://curative.com/pharmacy) including the Curative Pharmacy, Albertsons Pharmacy Companies (Including retailers such as Randalls and Safeway), H-E-B, Brookshire Brothers & Publix Pharmacy.

How do I access my prescriptions at the lowest possible cost?

How to qualify for $0 copay in three easy steps:

1. Get your Baseline Visit within 120 days of your start date.

2. Visit an in-network provider who prescribes a preferred medication.

3. Use our preferred in-network pharmacies.

__Preferred:__

Consists of generic, biosimilars*, select brand and specialty medication

$0 Copay

__Non-Preferred:__

Consists of lower value generics, brands, and specialty medications

$50/$250 Copay

*A biosimilar is a biological product that is highly similar to and has no clinical meaningful differences from an existing FDA-approved reference product. They are used to decrease healthcare costs and improve patient access.

Question 11

I need to refill a prescription in January and I have not been able to schedule my baseline visit? How do I transfer and fill my current my prescriptions?

1. Register your [member portal](https://health.curative.com) account

2. Transfer your prescriptions with the help of Member Services

3. [Schedule your Baseline Visit](https://health.curative.com/baseline) on the member portal or through member services.

4. Register your virtual care account to be ready when needed.

Question 12

How soon should I book my Baseline Visit?

Accepted Answer

We recommend booking your Baseline as soon as possible to ensure your appointment takes place within 120 days of the plan's effective date. Completing your Baseline will ensure you will continue to qualify for $0 copays and a $0 deductible for the plan year.

Question 13

How are Baseline Visits conducted?

Accepted Answer

Baseline Visits will be conducted virtually over Zoom. While we strongly recommend completing your Baseline Visit over a computer, if you join using a mobile device, you must download Zoom prior to your appointment. Members who access the Baseline Visit over a computer have the choice to join through a browser and not download Zoom.

It is strongly recommended that cameras are on during the Baseline Visit.

Question 14

Is there bloodwork at the Baseline Visit?

Bloodwork can be ordered if it is deemed medically appropriate.

What is a Care Navigator?

Your Care Navigator can help you find providers and navigate your benefits to help you reach your health goals. You do not need to wait until the Baseline Onboarding to connect with your Care Navigator. Go to your [Member Portal](https://health.curative.com/) to find their information.

Question 16

If I complete the Baseline Visit after the first 120 days, will the $0 copay and $0 deductible be reinstated?

Accepted Answer

It will not. The Baseline Visit must be completed within the first 120 days in order to unlock the $0 copay and $0 deductible benefits.

Question 17

What is the process, timeframe and general success rate there is for out-of-network providers to be brought in-network?

Accepted Answer

We actively try to bring out-of-network providers requested by members into the network. The process can take up to 120 days. A member requests a provider be in network. We pass it on to the recruitment team who gets in touch with the provider and send a contract. The most common delay in onboarding is a response from the provider, but we do everything we can to speed up the process.

Question 18

What if I am currently under doctor’s care for a chronic condition and really like my doctor but they are not in the Curative network?  How will that be handled?

Curative has a transition of care process for members under critical care. We ask that members contact member services or their care navigator to be routed to case management to discuss the process of transitioning to an in-network provider or qualifications to retain their current provider.
