How do I change or update the personal information listed on my member ID card(s)?

How do I add or remove a dependent?

All changes must go through your employer’s HR and benefits department, which will determine eligibility, cost adjustments, and the process for adding or removing dependents. Typically, changes can only be made outside of open enrollment during special enrollment periods or following a qualifying life event, as defined by your employer. In such cases, changes must usually be made within 31 days of the event, and coverage may be backdated to the event date or the first of the month. Please contact your employer to initiate any changes.

What is a Baseline Visit?

A Baseline is a chance for you to learn how to maximize your member benefits and health roadmap. All [Curative members](https://curative.com/for-members) start with no out-of-pocket costs for covered care and preferred prescriptions. To keep these benefits, you and any dependents 18 and older need to complete a Baseline any time in the first 120 days of the plan start. You will receive a welcome email with instructions 1-2 days before your plan start date. Once received, go to the Member Portal and register your account at health.curative.com. Once registration is complete, you will be able to schedule your Baseline in the [Member Portal](https://health.curative.com).

Who needs to complete a Baseline Visit?

Curative members who are 18 years old when their plan becomes active must complete a Baseline. If a member turns 18 later in the same year, they must complete the Baseline during their next plan year.

How often does the Baseline Visit need to be completed?

Accepted Answer

A Baseline needs to be completed every plan year and is vital for continuing access to Curative’s $0 copay, $0 deductible health plan. If you did not complete your Baseline in the first plan year, you will have another opportunity in the first 120 days of the next plan year. If you completed your Baseline in the first year, you are required to do a renewal Baseline in the 120 days from your next year's plan start date. Schedule and complete your Baseline yearly to sustain these benefits.

Do I need to complete my Baseline Visit before getting care?

No, you can get care as soon as your plan year starts. You do not need to wait for your Baseline. You will have $0 out-of-pocket for the first 120 days and need to complete the Baseline to continue $0 out-of-pocket costs for covered care for the remainder of the year.

Your appointment will be a Zoom video call and include a Curative Onboarding with a Care Navigator and a Clinical Check-in with a Curative Clinician. Upon renewal, members can choose to do a Self-guided Onboarding to get plan updates. on your schedule. The option is available via the Member Portal and you can book your Clinical Check-in once the Self-guided Onboarding is completed.

What happens if I or my dependent do not complete a Baseline Visit within 120 days of the plan start date?

If the Baseline is not completed within 120 days of your plan start date, you and your dependents lose access to the $0 copay and $0 deductible benefits, which could result in out-of-pocket expenses. Completing your Baseline on time ensures uninterrupted access to comprehensive, affordable care. Do I need to complete my Baseline Visit every year if I am a renewing member?

Yes, renewing members must complete their Baseline annually to maintain access to Curative’s $0 copay, $0 deductive offering. The Baseline helps ensure that your preventive care plan is current and tailored to your health needs. It’s an essential part of maintaining your health and maximizing your benefits. If you completed your Baseline the prior year, you will continue from the start with $0 out-of-pocket costs but need to complete your Baseline in the first 120 of the renewal start date. If you did not complete your Baseline the prior year, you will need to complete a Baseline in the first 120 days of the plan renewal date to regain $0 out-of-pocket costs.

Is this information shared with my employer?

No. Your Baseline is confidential, and under HIPAA guidelines, Curative will not share information with your employer. You also have complete control over what you share with the clinician so you can feel comfortable discussing your health privately.

No, completing your Baseline and the information shared does not impact your premiums.

Do I or my dependents need to prepare anything for my Baseline Visit?

While you don’t need to prepare anything, it is helpful to have the following on hand:

- __List of current healthcare providers:__ Include primary care physicians, specialists, or other healthcare professionals you regularly see.
- __List of medications:__ Include prescriptions, over-the-counter drugs, and supplements or vitamins you take regularly.
- __Current pharmacy:__ Note the pharmacy you use so we can confirm if it's a covered pharmacy.
- __Planned care/surgeries:__ List any upcoming procedures so we can check if prior authorization is needed.

Health concerns or questions: If you have anything on your mind—whether it’s a new	symptom or a general health question—write it down so you don’t forget to ask during your Baseline Clinical Check-in. Does this replace an annual wellness exam?

No, the [Baseline Visit](https://curative.com/baseline) is not a replacement for your annual wellness exam or your relationship with your primary care provider. It's designed to complement your existing healthcare routine and help you access resources and support.

Your Care Navigator can assist you in finding a primary care doctor if you don’t have one already. Additionally, if you're a member based in Texas, a Curative Clinician can become your primary care provider through [Curative Primary Care](https://curative.com/primary-care). Be sure to continue scheduling your regular physical examinations with your primary care provider for comprehensive care.

What is the Curative Cash Card?

The [Curative Cash Card](https://curative.com/cash-card) helps you access $0 care, even when your Member ID Card can’t be used. It’s designed for providers who accept cash payment instead of billing insurance. Use at providers marked as Cash Card on the provider search, or if the provider is on the provider search and won’t take your Curative Member ID Card. You pay nothing—Curative covers the cost directly.

Why is it called the Cash Card?

We changed the name from “Zero Card” to “Cash Card” to make it clearer and easier to use. It reflects how the card works—covering the cash-pay price at certain providers, so you don’t have to.

How do I get my Curative Cash Card?

Accepted Answer

The [Curative Cash Card](https://curative.com/zero-card) will be mailed to all members aged 18 and over who are enrolled in the Curative Health Plan at the start of the plan year. Accessing your card is as easy as logging into your [Member Portal](https://health.curative.com). You can also download your card and add it to your digital wallet:

1. Open the Google Pay app.
2. Tap "Add to Wallet" at the bottom.
3. Select "Payment card."
4. Tap "New credit or debit card."
5. Tap "Save" at the bottom.
6. Read the Issuer Terms and tap "Accept."
7. If prompted to verify your payment method, choose an option from the list.

__For Apple Wallet:__
1. Open the Wallet app on your iPhone.
2. Sign in with your Apple ID if required.
3. Tap the "+" sign and choose "Debit or Credit Card."
4. Enter card details manually or position your physical card in the frame to scan.

When should I use my Curative Cash Card?

- When a provider shows as “Cash Card” in our [Provider Search](https://curative.com/providers). 
- When an in-network provider on the Provider Search doesn’t accept your Curative Member ID Card or tries to charge a copay.

Who is eligible to use the Curative Cash Card?

In the first 120 days, the [Curative Cash Card](https://curative.com/cash-card) is made available to all members 18 and over. For the remainder of the year, the Curative Cash Card is only available to members 18 and over who have completed their [Baseline Visit](https://curative.com/baseline). Cardholders can use it for themselves or their covered dependent(s).

Why does Curative have two different cards for members?

Our dual-card system allows more provider choice and less friction with no out-of-pocket costs for members who complete a Baseline Visit within the first 120 days. Providers will either bill through an insurance claim or take self pay at the point of service. Either way, members pay $0 copays, $0 deductibles and Curative foots the bill. 
- __The Curative Member ID Card__ is the traditional route – providers bill through claims and Curative pays those claims.
- __The Curative Cash Card__ is on-the-spot payment – it allows members to see providers that don’t take the Member ID Card for any reason. Some providers may not take insurance at all, like many mental health providers, or they may not recognize Curative as contracted but do appear in the provider search. Some providers are designated as Curative Cash Card only and you should show the Curative Cash Card first.

Are the cardholders responsible for any expenses? Does it impact credit?

Members will not owe anything as long as the provider is Curative Cash Card approved, as designated in Curative's [provider search](https://curative.com/providers), and it is used for a covered service. Curative pays for all approved charges and member credit is not impacted.

What services does the Curative Cash Card cover? Does it include prescriptions?

We recently made some improvements to the Curative Cash Card.

__What’s new:__
- Members can now use the Curative Cash Card for inpatient and outpatient hospital services at most hospitals
- No prior authorization is required for these services when using the Curative Cash Card.
- This applies to all members, regardless of their benefit plan—EPO and PPO included.

__How it works:__
- Members present their Curative Cash Card at check-in and state they are paying cash.
- Curative covers the cost directly; members pay $0 out of pocket.
- Our Member Services team is standing by to support members with any access issues.

__Eligible services with the Curative Cash Card include:__
- Office visits
- Urgent care
- Behavioral health
- Outpatient hospital services (like imaging or procedures)
- Inpatient hospital services (like surgeries and medical monitoring)

Why do you guarantee your provider search?

Accepted Answer

Provider searches are often inaccurate across the healthcare industry, but we believe members shouldn’t be penalized.

EPO, PPO, and PPO Max members: If a provider is listed in Curative’s Provider Search and you receive a covered service (with prior authorization if required), Curative guarantees $0 out-of-pocket costs—provided you’ve completed your Baseline Visit within the first 120 days of coverage.

EPO Value members: The Provider Search is informational only. You can always view our full in‑network directory on our provider search tool, but EPO Value covers $0 care only with providers selected through your Curative Pass.

*Reminder: Nominating a provider for the Curative Cash Card does not automatically guarantee their acceptance. All nominations must go through Curative’s contracting and credentialing process.

Question 30

What if I have a favorite doctor and don’t see them in the provider search?

Members can easily nominate providers for the [Curative Cash Card](https://curative.com/cash-card). All they need to do is fill out a quick form at [cur.tv/nominate](http://cur.tv/nominate). Members will hear back within five business days, and, if approved, can see that provider immediately using the Curative Cash Card. Curative will then consider adding that provider to the network, which can take much longer.

What are the criteria for the Curative Cash Card approved providers? How is that different from an in-network or out-of-network provider?

Accepted Answer

An out-of-network provider [can be nominated](https://form.typeform.com/to/NWyaOFHz) to become a Curative Cash Card-approved provider. Our team will review for quality and cost measures before approving. Out-of-network providers do not undergo any review process by Curative. On the other side of the spectrum, in-network providers are contracted through Curative or the rented First Health network for specific rates, quality guidelines, and to request certain practices for improved care coordination.

What do providers think of the Curative Cash Card?

Many providers accept self pay as an option and appreciate the ease and ability to get paid on the spot instead of dealing with insurance billing. The cash price is set by the provider.

Will cardholders receive receipts or records of visits charged to their Curative Cash Card?

Yes, we will provide a statement much like a standard Explanation of Benefit.

I have a Curative Zero Card. Do I need to get a new Curative Cash Card now that the name has changed?

No. Your existing card still works exactly the same.

Can I suggest a provider to be added to the Curative Cash Card approved list?

Yes! If you have a provider who doesn’t currently accept the Curative Cash Card, you can [nominate them](https://form.typeform.com/to/NWyaOFHz). Note: not available to EPO Value members.

Why is the Curative Cash Card needed at some hospitals that previously took the Curative Member ID Card?

These hospitals are currently not part of our traditional in-network contracts. But with your Curative Cash Card, you can still receive $0 care there, without delays or extra paperwork. Just present your card at check-in and let them know you’ll be paying the cash price.

Can I use Curative Cash Card for prescriptions or labs?

No. The Curative Cash Card can’t be used for prescriptions or labs. Your Curative Member ID Card is still needed for those.

What is a claim?

A claim is a request for payment that your healthcare provider sends to your health plan. All the information your doctor needs to submit a claim to Curative is on your insurance ID card.

How do I submit a claim from an out-of-network payment made to my physician if I haven’t received my ID card yet?

You can access your digital ID card in [Curative’s Member portal](https://health.curative.com/) and save it to your mobile phone or submit a claim form. To submit a claim form for an out-of-network payment made to your physician before receiving your ID card, follow these steps:

1. __Obtain a Claim Form:__ Visit the Curative website to download the claim form, or contact our member services team to request one.
2. __Complete the Form:__ Complete the claim form with your details and information regarding the services received, including dates of service and the provider’s information.
3. __Attach Documentation:__ Include any receipts, invoices, or proof of payment you received from your physician.
4. __Submit Your Claim:__ Send the completed form and supporting documents to the address specified on the claim form or submit them via the [Curative member portal](https://health.curative.com/).

Does Curative cover medical care received while traveling abroad?

Currently, no services outside the United States are eligible for coverage unless it is a medical emergency. If a member is traveling outside of the United States, buying a travel insurance plan is in their best interest. My claim was denied; is there an appeal process?

If you disagree with a prior authorization or claim denial, you have the right to request an appeal. For claims, you must submit your appeal within 6 months from the date of the claim. For prior authorization denials, appeals must be submitted within 120 days of the denial.

Curative guarantees its [Provider Search tool](https://curative.com/providers). Provider searches are known throughout the industry to be inaccurate and outdated, but we believe members shouldn’t have to pay the price. If the provider is in the directory and it is for a covered service, we will cover it with either your [Curative Cash Card](https://curative.com/cash-card) or Member ID Card.
