Meet Zero First: $0 for most, priced to compete
Zero First is a Curative plan that pays first. Care is free up to a set amount each year, then members pay a known share of each bill, never more than their out-of-pocket max. Preventive care, the Baseline Visit, primary care, outpatient mental-health visits and urgent care are $0 all year.
Priced for a routine year. Capped for a hard one.
Most people’s years are routine: checkups, a prescription, a visit or two. Employers and their employees shouldn’t have to pay for a worst-case scenario every year. Zero First keeps what people love about Curative at a more competitive price.
Not a copay, not a deductible, something better
How Zero First works
Free up to a set amount
Specialist visits, tests, imaging and hospital care are $0 until a person's in-network care reaches the set amount: $2,500 on Zero First EPO, $5,000 on Zero First PPO.
Above it, a known share of each bill
After the set amount you pay a flat share of each in-network bill: 30% on Zero First EPO, 25% on Zero First PPO. Providers bill the plan first; your share comes on your Explanation of Benefits.
Never more than the out-of-pocket max
Payments stop at the out-of-pocket max: $8,000 on Zero First EPO, $6,350 on Zero First PPO, medical and pharmacy copays together. After that, everything in network is $0 again.
Some care is $0 all year
Preventive care, the Baseline Visit, primary care, outpatient mental-health and substance-use visits and urgent care are $0 all year and never count toward the set amount. Prescriptions do not change.

Prescriptions stay simple
Pharmacy benefits don't change: $0 preferred, $50 non-preferred, and $250 non-preferred specialty, with 97% of fills at $0. Prescriptions never count toward the set amount, and any copays count toward the out-of-pocket max, so medical and pharmacy share one out-of-pocket max.

Zero First is available to level-funded groups effective 1 December 2026 or later. Preventive care, the Baseline Visit, primary care, outpatient mental-health and substance-use visits and urgent care are $0 and never count toward the set amount. Urgent care means services billed with the S9083 urgent care global code. Pharmacy benefits are unchanged; prescriptions do not count toward the set amount, and any pharmacy copays count toward the out-of-pocket max. The Baseline Visit must be completed within 120 days of the plan effective date. Fully insured availability subject to state approval. Your group's numbers are on your quote; plan documents govern.
