For Curative members only. A ticket to get in-network healthcare where and when you need it. As the fresh face in health plans, we know some doctors might not recognize us just yet. No worries, though! Your health shouldn't have to wait. This handy card takes care of your in-network costs and reassures you of your benefits – if the provider is listed in the directory, we will cover it.
Getting started
Check to make sure your provider is in the search tool. If not listed, call member services at 855-428-7284 for approval. Keep in mind, zero card only providers will only be approved through your zero card, not your plan id card.
Get a digital/physical Curative Zero Card
Activate the card through the member portal
Don't see your preferred provider? Nominate your go-to provider to become zero-card eligible.
Simply give the Zero Card to the provider's office for payment and you're all set!
Access the Member Portal
Open the wallet app and sign in with your Apple ID if required.
Tap the "+" sign and choose "Debit or Credit Card."
Enter the Curative Zero Card details manually or position your physical Curative Zero Card in the frame to scan
Access the Member Portal
Open the Google Pay app and tap "Add to Wallet" at the bottom. Then, select "Payment card."
Tap "New credit or debit card" and tap "Save" at the bottom.
Read the Issuer Terms and tap "Accept." If prompted to verify your payment method, choose an option from the list.
The Curative Zero Card is available to all members during their first 120 days on any Curative Health plan, and for the full year to members who have completed their Baseline Visit during the first 120 days. Curative Zero Card Visa® Commercial Credit cards are issued by Celtic Bank. Additional Terms & Conditions can be found in your Member Portal Account.
