How do I know if my doctor is in- or out-of-network?

For EPO and PPO members, if your clinician appears on the provider search, they are covered at $0 using either the Curative Member ID Card or Curative Cash Card, as specified. If your clinician is not on the provider search, they are out-of-network.

For PPO Max members, you can use your Curative Member ID Card to see an out-of-network provider for $0 copays and deductibles but may be subject to balance billing. You can also nominate your provider to be covered for certain services with the Curative Cash Card. Learn more about the Curative Cash Card and how to nominate.

EPO Value members may view our full in‑network directory on our provider search, but EPO  Value covers $0 care only with providers selected through your Curative  Pass.

What is a Baseline Visit?

A Baseline is a chance for you to learn how to maximize your member benefits and health roadmap. All Curative members start with no out-of-pocket costs for covered care and preferred prescriptions. To keep these benefits, you and any dependents 18 and older need to complete a Baseline any time in the first 120 days of the plan start. You will receive a welcome email with instructions 1-2 days before your plan start date. Once received, go to the Member Portal and register your account at health.curative.com. Once registration is complete, you will be able to schedule your Baseline in the Member Portal.

Does my Curative health plan cover telehealth care appointments?

Yes, telehealth visits are covered under your Curative health plan. In many cases, they’re included at $0 out-of-pocket cost when using providers listed in the provider search. Check your plan details or contact Member Services at 855-428-7284 or health@curative.com for specifics.

What is the Curative Cash Card?

Accepted Answer

The Curative Cash Card helps you access $0 care, even when your Member ID Card can’t be used. It’s designed for providers who accept cash payment instead of billing insurance. Use at providers marked as Cash Card on the provider search, or if the provider is on the provider search and won’t take your Curative Member ID Card. You pay nothing—Curative covers the cost directly.

When should I go out-of-network using my Curative Member ID card, and when should I use my Curative Cash Card?

The Curative Member ID Card and Curative Cash Card are two different ways to pay a provider. If the provider is out-of-network, use the Curative Member ID card. Curative will cover the copay and deductible, but you may receive a balance bill. Use the Curative Cash Card for any provider marked Curative Cash Card in our provider search.
