Everyone has a distinct health journey, and it is essential to have a plan that meets your individual needs. Your employer may choose a health insurance plan, but you often have the option to select a specific product that aligns with your health goals. All Curative plans emphasize preventive care services by bringing copays and deductibles down to $0 as long as a Baseline Visit is completed in 120 days. Not all health insurance products cover preventive health services, and new rulings could soon lower the threshold for preventive care coverage. But as a Curative member, you can can rest assured you’ll have access to high-quality in-network care that prioritizes preventive health — no matter which plan you choose.
Before diving into the Curative plan options, it's crucial to comprehend the basics of an Exclusive Provider Organization (EPO) plan and a Preferred Provider Organization (PPO) plan, in general.
Breaking down the basics: EPO vs. PPO
EPO and PPO plans are two of the most common types of health insurance plans. Both plans help pay for the medical care you need but differ in terms of scope and coverage.
Under an EPO plan, insurance companies help pay for members’ health care if it’s from in-network doctors and hospitals once members reach their deductibles. Members must seek care from in-network providers to avoid additional medical costs with an EPO plan. If a member goes to an out-of-network provider, they’re responsible for the full cost. Notably, there are exceptions in case of emergency and if a member needs certain care services that aren’t available in-network. The limited network can make EPO plans a pricey option if you need to use out-of-network providers.
Yet, EPO plans remain a popular choice for members because they generally feature more affordable monthly premiums, depending on your employer-sponsored coverage, and lower out-of-pocket costs than PPO plans.
On the other hand, PPO plans cover a percentage of both in-network and out-of-network care once deductibles are met. PPOs offer lower out-of-pocket costs for in-network services, and although they have higher premiums than EPO plans, this might not affect you depending on how much your employer contributes. If the choice of out-of-network care is important to you, a PPO plan might be worth the investment.

While the EPO plan does not provide benefits for out-of-network services, there are exceptions for:
Emergency services
Services from facility-based providers in a network facility
Laboratory service providers when services are provided in connection with a service from a network provider
Diagnostic imaging services from an out-of-network provider when services are provided in connection with a service from a network provider
PPO
The classic package. After the Baseline Visit, members can start using their plan with:

$10,000/person, $20,000/family out-of-network deductible
$15,000/person, $30,000/family out-of-network max out-of-pocket

PPO+
A little extra. Curative’s PPO+ plan brings you all the classic PPO benefits with a lower threshold to afford out-of-network providers and some extra coverage. After the Baseline Visit, members can start using their plan with:
$0 in-network deductible
$5,000/person, $10,000/family out-of-network deductible
$7,500/person, $15,000/family out-of-network max out-of-pocket
Access to a personal Care Navigator

Wellness benefits, including chronic condition and acute care management, and preventive health services
Access to ClassPass
Chiropractic coverage
As a Curative PPO+ member, you can unlock a 25-credit monthly membership with ClassPass at no cost to you. ClassPass is a credit-based membership designed to bring you access to thousands of studios, gyms, salons, and spas all through one app.
Curative PPO+ also covers chiropractic care for the treatment of muscoskeletal disorders, up to 20 visits in plan year.
As the product develops, more features — like additional coverage — may be added.
