Instead of browsing the full provider directory, EPO Value members use the Curative Pass tool in the Member Portal. This tool generates a short list of in-network providers based on your needs and location. When you choose a provider from the list, your Curative Pass is created instantly.

As a new member on EPO Value, can I keep my provider?

Accepted Answer

We can’t guarantee that your current provider will appear in your Curative Pass list. If staying with a specific doctor is important, consider choosing the EPO, PPO, or PPO Max plans instead.

Can I request a new list of providers if I can’t find an appointment or the options don’t meet my needs?

Yes. You can request a new list of in-network providers any time using the Curative Pass tool.

Can I see any in-network provider listed in the provider search tool?

EPO, PPO, and PPO Max members: Yes. You may see any in-network provider listed in the Provider Search.

EPO Value members: You can always view our full in‑network directory on our provider search tool, but EPO Value covers $0 care only with providers selected through your Curative Pass.

How long does a Curative Pass last?

Each Pass is valid for a specific period, depending on the type of care. If your appointment is outside the window or you need additional visits, you can request a new Pass.

Do I need a new Pass for follow-up care or multiple visits to the same provider?

Accepted Answer

Yes. Each visit requires a valid Curative Pass. If your follow-up is outside the original window, request a new Pass for continued $0 coverage. In some cases, you may be able to renew a Pass with a specific provider.

I secured a Curative Pass to see a provider, and now I plan to seek additional care at a separate facility within the same facility system. Do I need a second curative pass?

Accepted Answer

Yes. A Curative Pass is required for each location (physical address) where you receive care.

Do I need a Curative Pass for prescriptions or pharmacy visits?

Accepted Answer

No. Prescriptions work the same across all Curative plans.

$0 prescriptions are available through mail order and select retail pharmacies like H-E-B, Albertsons, Safeway, and Publix (30,000+ locations).

A searchable drug list is available at curative.com/drugs, including many $0 options.

Does urgent care or emergency care require a Curative Pass?

No. Emergency care is covered with your Member ID Card—no Pass needed. Urgent care is also covered under the Member ID Card or automatically under the Curative Cash Card for eligible services.

What happens if I see a provider not listed through the Curative Pass tool?

You can still see in-network providers, but you’ll likely owe copays, deductibles, or coinsurance. The $0 benefit only applies when you use the Curative Pass.

Do I still need to complete a Baseline Visit on this plan?

Accepted Answer

Yes. All Curative members must complete a Baseline Visit within 120 days of their plan start date to keep $0 benefits active. Without a completed Baseline Visit, your plan will default to standard cost-sharing, and you’ll lose access to $0 care—even with a Curative Pass.

The Curative Cash Card expands access to $0 care in certain situations:

It works automatically at urgent care locations for eligible services.
For all other care, you must first get a Curative Pass. If the provider on your list only accepts cash payment, Curative will activate the Cash Card for that appointment.
