Curative makes care accessible across the U.S. through our proprietary network and a strong, supplemental national provider network through the First Health network. For EPO Value Members: First login to the Curative Member Portal to obtain a Curative Pass and find providers.

Finding your favorite care providers is important to your long term health. EPO, PPO, and PPO Max Members: Use our provider network search tool. EPO Value Members: Sign in to the Member Portal and open the Curative Pass tool to search for the care you need. You can also reach out to your Care Navigator or Member Services at 855-428-7284 and they will be happy to serve you.

We know how important it is to have a provider you trust. Here’s how to connect with the right care based on your plan type:
EPO Value Members: The Curative Cash Card gives you access to $0 care at any provider labeled “Curative Cash Card Only" that you have a Curative Pass for instead of your Member ID card. It can be used as a backup when you have a Curative Pass but your provider doesn’t take your Member ID card.
All other Curative plan members: You can use the Provider Search tool to find high-quality, in-network providers for $0 care.
If your provider isn’t already in network, you can nominate them to join the Curative Cash Card network using the form below.
