What happens to your copay or deductible when just one adult enrolled in the plan completes the Baseline Visit? If all adults enrolled complete the Baseline, do minor dependents also get the $0 copay/deductible?

Every plan member 18+ years at the plan activation date must complete the Baseline Visit to qualify for $0 copays and a $0 deductible for the year. If you have a child dependent on the plan (0-17 years old at the start of plan coverage), either the subscriber or spouse enrolled in the Curative health plan must complete the Baseline Visit for your child to qualify for $0 copays and a $0 deductible for the year. If your child dependent turns 18 in the year after the plan activation date, they will continue to see no cost sharing and do not need to complete a baseline visit for that initial plan year.

How is a Baseline Visit different from a traditional physical?

Your Baseline Visit will have two main parts, a clinical portion and an onboarding portion with your Care Navigator. The clinical portion is not to be confused with or take the place of a routine physical with your primary care physician. The clinical portion of the Baseline Visit is an opportunity to participate in routine screenings, including point-of-care labs, and to receive select immunizations if in-person. The clinician will make sure you are set up with the proper care you need and make preventive recommendations based on your current health snapshot. In the onboarding portion, your Care Navigator will explain the full details of your health plan, including a breakdown of your benefits and how to use them. Care Navigators can assist members in finding a primary care physician, transferring your prescriptions, and ensuring you understand your coverage. Your Care Navigator will be your personal point of contact that will support you throughout your entire journey as a member.

During your Baseline Visit, you will meet with your Care Navigator, who will explain the full details of your health plan, including how to best utilize your benefits. Care Navigators can assist members in finding a primary care physician, transferring their prescriptions, and ensuring they understand their coverage. Your Care Navigator will be your personal point of contact that will support you throughout your entire journey as a member.

Your Care Navigator is your go-to source for all things Curative, and will be your direct point of contact if you have questions or concerns about your coverage. Your Care Navigator can help you find providers and navigate your benefits to reach your health goals.

Question 4

How soon should I book my Baseline Visit?

Accepted Answer

We recommend booking your Baseline Visit as soon as possible in order to ensure your appointment takes place within 120 days of plan effective date, which ensures you will continue to qualify for $0 copays and a $0 deductible for this year.

Question 5

How long does a Baseline Visit typically take?

Accepted Answer

We know your time is important. Most Baseline Visits take between 45 minutes to 1 hour to complete. Please allocate enough time for travel if completing an in-person visit or for any potential technical difficulties if joining virtually.

Question 6

What do I need to bring to my Baseline Visit?

Accepted Answer

Please come to your Baseline Visit with any questions you might have about your Curative plan, as well as any health goals or concerns you’d like to discuss. It will also be helpful to have your Member Portal login information so that we can walk you through the portal. Our Care Navigators will be prepared to help you find answers to your questions and set you up to utilize your Curative benefits!

How are Baseline Visits conducted for members outside of Texas?

Baseline Visits will be conducted virtually over Zoom. While we strongly recommend completing your Baseline Visit over a computer, if you join using a mobile device, you must download Zoom prior to your appointment. Members who access the Baseline Visit over a computer have the choice to join through a browser and not download Zoom.

Do I have to have my camera on during my virtual Baseline Visit?

It is strongly recommended that cameras are on during the Baseline Visit.

Question 9

If I’m in the Austin area, do I have to come in for my Baseline Visit, or can I do it virtually?

Accepted Answer

You have the option to complete your Baseline Visit virtually, but we highly recommend that members come in to the Curative Commons at 900 Congress Ave, Austin, TX 78701 if possible for the best experience and welcome gifts.

Question 10

What is Curative doing with the data collected during my Baseline Visit?

Data collected for or during your Baseline Visit will not affect rates or premiums for insurance in any way, and information will not be shared with your employer. We want to make sure that members get the best care. Collecting certain health information allows us to better understand our members' needs, so as to improve their patient experience.

If I am under 18, do I need to complete a Baseline Visit?

Only Curative members who are 18 years of age at the date of their plan activation will need to complete a Baseline Visit. Members who turn 18 later that same year will not need to complete a Baseline Visit until their next enrolled year.

Question 12

How are virtual and in-person Baseline Visits different?

Accepted Answer

Virtual Baseline Visits do not collect blood pressure measurements, on-site lab work, height and weight measurements, or offer vaccines; instead, they will review your medical history and make age-appropriate wellness and preventive recommendations. All in-person visits come with welcome gifts and the opportunity to meet with your care team face-to-face. We recommend the in-person Baseline Visit for all who are able to make it in to the office.

If I have a pre-existing condition, will there be a waiting period for me to utilize my insurance or could I possibly be denied coverage?

There is no waiting period before you’re eligible to use your insurance, and we will not deny coverage based on any pre-existing conditions.

Will my employer get the results of my Baseline Visit?

Accepted Answer

Your Baseline Visit is confidential, and under HIPAA guidelines, no information will be shared with your employer.

Question 15

What clinical aspects are included in the Baseline Visit?

Accepted Answer

In-person clinical visits include point-of-care lab tests, including a blood sugar level test and lipid panel. Members will also have the opportunity to opt for select vaccines they may be missing. All clinical visits, both virtual and in-person, include time to discuss any health questions or concerns you have. The clinician will also ask you some current and past medical questions to make sure you are set up with all the care and resources you need. Members can opt to not respond to any of the questions asked by the clinician. Completing a Baseline Visit should not replace your relationship with your primary care provider or take the place of your annual physical.

What can I expect to happen after my Baseline Visit?

After the Baseline Visit, you can expect a follow up email or phone call from your Care Navigator to ensure we’ve addressed all outstanding items. The follow-up email will include a reference sheet with convenient in-network providers, facilities, and pharmacies, along with contact information to reach your Care Navigator and other plan resources. You can discuss with your Care Navigator how you’d like to communicate moving forward. If there are any concerns, we will refer you to our Healthy Connections team, where a case manager will follow up with further support.

What questions are asked during the Baseline Visit?

Your Care Navigator will ask you about any health concerns you may have, along with your health goals. They will ask if you have any questions about the Curative plan, and help you locate your nearest physician, pharmacy, urgent care facility, and hospital. If participating in a clinical visit, the clinician will ask about your medical history or any medical concerns.

What if I can’t find a time that is available and convenient for my Baseline Visit?

You have 120 days from the date of your plan start date to complete your Baseline Visit. If none of the available times work for you, you can reach out to Member Services at 855-428-7284 and we can see if there are any accommodations we can make. Please note that we offer both virtual and in-person Baseline Visits. If you looked for an appointment previously and there were none available, more appointments may open up due to cancellations or extended capacity, so please continue to monitor the member portal.

How is the Baseline clinician connected to the insurance company?

The Baseline Visit is conducted by an independent physician entity.

Question 20

Will my doctors receive any results from this visit?

Accepted Answer

Your doctors will not automatically receive results from your visit. However, members are given a print out of their clinical results for all in-person appointments, which they can choose to bring to a provider.

What happens if you find something serious about my health?

If something serious is discovered during this appointment or through the clinical labs, we would immediately refer you to the appropriate level of care.
