Sabemos lo importante que es contar con un proveedor de confianza. A continuación, le indicamos cómo conectarse con la atención adecuada según su tipo de plan:

  • Afiliados a EPO Value: la Curative Cash Card le da acceso a atención médica sin costo con cualquier proveedor que indique “solo Curative Cash Card” y para el que tenga un Curative Pass en lugar de su tarjeta de afiliado. Se puede utilizar como respaldo cuando tiene un Curative Pass, pero el proveedor no acepta su tarjeta de afiliado.
  • Todos los demás afiliados del plan Curative: puede utilizar la herramienta de búsqueda de proveedores para encontrar proveedores de alta calidad dentro de la red que presten atención médica sin costo alguno.

Si su proveedor aún no forma parte de la red, puede proponer su incorporación a la red Curative Cash Card mediante el siguiente formulario.
