Your right to medical records
Under state and federal law, patients (or their parent/legal guardian) have the right to inspect and obtain a copy of their medical records.
How to request your records
You may request your medical records by submitting a completed Medical Records Request Form:

Requests must be complete and signed to be processed.
Response time
Records will be provided within 15 business days of receiving a complete request, as permitted by applicable state law.
Your rights
You have the right to:
Receive copies of your medical records
Request records in electronic format, when available
Request an amendment or correction to your record
Receive a written explanation if access is denied or limited
Access may be limited in certain circumstances as permitted by law.
