COVID-19 testing could be reimbursed by your health plan or the government, with no out-of-pocket cost, when billed to your health plan or the government, upon verification of your insurance/eligibility information. Curative Labs Inc will submit a claim to your active health plan on your behalf ranging from $250.00 to $300.00 (our published cash price, depending on the test performed).The claims process can be lengthy and confusing, and we want to make it as easy for you as possible. The information below may be helpful.Explanation of benefits (EOB) from your insurance company: You may have already received a written summary of the claim we submitted and its status from your insurance company. This document is called an Explanation of Benefits (EOB). Your insurer may send a payment directly to you rather than paying Curative Labs Inc. If you receive a check from your insurance company for COVID-19 testing, please forward the payment to Curative Labs Inc by endorsing the back of the check with your signature and the words "Pay to the order of Curative Labs Inc."Then mail the signed check and a copy of the EOB to:

Curative is aware that patients may receive an Explanation of Benefits (EOB) for testing or vaccinations from their insurance companies. An EOB is NOT a bill. Curative will NOT “balance bill” you for COVID-19 testing/vaccine services.
If you receive an Explanation of Benefits (EOB) or other paperwork from your insurance company, this is NOT a bill for Curative services. The document shows a review of services performed by Curative, billed to your insurance company. The report shows the amount the insurance company paid to Curative and the balance not paid. The insurance company may identify balances as “Amount not Covered” “Amount you May Owe” “Not Covered” and/or “Member Responsibility” in the EOB report. As per the CARES Act, you are not responsible for this balance, or any charges related to COVID-19 related services that an insurance company does not cover. You will not receive a bill from Curative requesting payment for the balance not paid by your insurance company unless you are paid directly by your insurance (see above instructions). Please do NOT pay any balances the EOB may say are owed to Curative. While Curative cannot control how your insurance carrier will process your insurance claim or require insurance companies to refrain from issuing EOBs for COVID-19 related services (as this is standard practice), please note that Curative will not seek any payments from balances shown on patients’ EOBs.
COVID-19 testing is NOT reimbursable by private insurance (outside the state of California) for purposes including, but not limited to, the following:
Epidemiological purposes
Surveillance
Community tracking
Employment purposes, including staffing decisions
Determine the need for personal protective equipment
Screening (e.g., travel, sports, school, camp, recreational, social requirement)

California Law. With the recent passage of SB 510, California law makes clear that California health plans are required to pay the provider for COVID-19 diagnostic and screening testing as well as other services (the “California COVID-19 Coverage Law”): [A] health care service plan contract that covers medical, surgical, and hospital benefits, excluding specialized health care service plan contract, shall cover the cost for COVID-19 diagnostic and screening testing and health care services related to diagnostic and screening testing approved or granted emergency use authorization by the federal Food and Drug Administration for COVID-19, regardless of whether the services are provided by an in-network or out-of-network provider. [1]The new California COVID-19 Coverage Law applies to all testing performed since March 4, 2020. [2] It also expressly requires health plans to “reimburse the provider of the testing”.
